Hi,
Since you mentioned you only have the one project, this is happening within a single project's Board view rather than across several.
Two more things would help us pin it down:
- Which Shuffle board option you're using.
- Whether you've applied any grouping or sorting in the filter bar. A screenshot of that would help us see your exact setup.
Knowing your exact setup will help our team investigate further.

- How you're adding the task (the + button, a keyboard shortcut, or pressing Enter)
- After you reload, can you create tasks again, or does it keep happening?
- Does it happen in every project, or just one?
- Are you on the web app or mobile, and if web, which browser?
- Roughly when it last happened, with the date and time if you have it
Once we have that, our team can look into what's causing it.

In the board view, using the + menu at the top right, shortcut, and and + on the top of a swimlane
