Risk statement. The schema migration rewrites 14 million customer records in place. The rollback path has been written but never executed against production-scale data, and the restore time from backup is untested.
Trigger / early warning. Any dry-run migration producing a row-count mismatch or failing its integrity check.
Consequence if realised. Catastrophic and effectively unrecoverable. Low likelihood, but this is the one on the register that could end the project.
Current response. Mitigate. Rehearse the full migration and rollback on a production-sized copy, and verify a timed restore before touching production.
