Monthly re-scoring of the full register

Cadence: first working day of the month, 60 minutes. Owner: project manager with risk owners present.

Weekly review covers the top of the register. This one covers all of it, including the Low risks nobody has looked at.

What to do:

• Re-score every open risk. Likelihood in particular drifts as the project progresses; risks tied to early phases should be falling.
• Close anything that can no longer occur. A register that only grows is a register nobody trusts.
• Check that every risk still has a named owner who is still on the project.
• Confirm each risk has a Response Strategy set. Blank is not a strategy.

Output: an updated register, plus a short note of what changed for the steering pack.
