Cadence: quarterly. Owner: project manager.

One page. Five things:

1. Top five risks by score, with the trend arrow since last quarter.
2. Risks closed this quarter, and why.
3. New risks identified this quarter.
4. Contingency drawn down to date, against the reserve.
5. Decisions needed from the steering group, each with a recommendation attached.

Do not present the full register to a steering group. They will read the first three rows and stop.
