Add a "QA-ready by day 7" checkpoint to the board

New board column plus a simple rule: if a story is not in QA by day 7, it gets raised at the mid-sprint checkpoint. Not a hard gate, just a visible signal so the pile-up is obvious on day 7 instead of day 10.
